If you confirmed your eligibility using the state's Phase Finder tool, you must still schedule an appointment with a vaccination site or submit a request form on the Public Health website to be referred to a facility with vaccine. Phase Finder only determines eligibility; it does not schedule you an appointment or add you to a waiting list.
