Hospice care provides medical, emotional, and spiritual care for patients in their last months of life. Hospice care is a family affair and the patient"s family is encouraged to be involved in their loved one"s plan of care. A team of hospice specialists including doctors, nurses, social workers, chaplains, aides, and trained volunteers helps the patient stay comfortable. Working together with the family and their primary doctor, the team adds an extra layer of care for the patient.

Community Home Health & Hospice offers hospice care at home (wherever your loved one lives) and in our care center. Short-term respite care is also available when a family member needs a rest from caring for their loved one at home.
